General FAQs
How do I contact the clinic?
The office number is (205) 982-3596. Our hours are 7:00 am to 5:30 pm Monday through Thursday. If the office is closed, we prefer that you call back during business hours. If you have a medical emergency or an urgent problem, we have a clinical staff member after hours. Please be aware that the on-call service is only for emergencies. No medication changes will be made after hours or on the weekends.
Where are you located?
We are located 1 mile from the I-65 exit on Valleydale Road in Birmingham, AL. Please see this link for directions.
How do I schedule an appointment? Do I need a referral?
Please call our office at (205) 982-3596 for an appointment. You do not need a referral; however, we will ask you to provide 3- 6 months of medical records documenting your care for your pain problem and any diagnostic imaging reports. Physicians can indeed make referrals by fax at 1-877-399-1545.
What kinds of pain do you treat?
All types of non-cancer chronic pain are treated. We are not an acute pain care clinic. Conditions treated include but are not limited to: chronic low back pain, chronic neck pain, arm and leg pain, herniated discs, arthritis, fibromyalgia, migraines and headaches, reflex sympathetic dystrophy (RSD or CRPS), diabetic neuropathy, joint pain, pelvic pain, abdominal pain.
Does my insurance cover your doctors?
Medicare and most major insurance companies cover medical treatment at a partial rate. "Mental/nervous" coverage (for our required psychology visits) varies. Coverage will need to be confirmed with your insurance company. We do accept self-pay patients on a fee-for-service basis. We participate with many insurance payors and will bill all insurance companies as a courtesy to our patients. You are required to pay all copays and deductibles at the time of service.
Why does it take so long to get the appointment with your medical doctor for pain management?
When your doctor makes the referral to us, we review your records and may need to request additional records. We also verify insurance. That process can take up to a few weeks, depending on the amount of records. We then schedule for an evaluation with one of the clinic psychologists, who will take a complete history and discuss with you the clinic requirements. We now have two full-time physicians, two nurse practitioners, and are running at full capacity; thus, your waiting time will be as short as possible.
